ID Information Cards
for CLS Employees
If you are a CLS employee you can get the following cards from the office:
Photo ID Card
- This is a laminated card containing your signature, photo, name, job title, as well as the CLS logo/name, Administration Office: address, phone number, fax number, website & email address. Your picture will be taken at the office. How to obtain one.
InfoCards
- These cards are for emergency use while supporting out in the community. They contain the following text plus the employee's first name. How to obtain one.

I am name, employed by the Community Living Society
and trained to deal with this situation in a professional
manner. I'm doing my best to handle this situation for
the safety and benefit of the individual, and those nearby.
We believe that all individuals are entitled to have access
to the community, and we are here to support them to
do that.

Business Cards
- These cards contain the company logo/name, Admin. office address, phone/fax numbers, website, email address and the employee's name, title, work location address, phone/fax numbers, and if applicable, the employee's pager number and voicemail number.
How to get your card(s)
- If you would like a photo ID card, call the Office Manager and let her know when you will next be in the office and give her your primary work location.
- If you feel you need an Information card and/or business cards, please speak to your Supervisor. You Supervisor will then contact the Office Manager.
